It heats the back of my neck and my shoulders, along with the muscles on the front of my chest all at the same time. I'm a plus-size female and it fits me just fine. There are low, medium, med-high and high settings and I personally like the highest setting for my tough muscle aches. I sit at a computer all day so I often get stiff and achy in my neck and shoulders at the same time. Nothing else fit quite like this does, and it stays in place pretty well. The fabric cover helps prevent slipping off, but can be easily removed for washing.As for the reviewers that say this product doesn't get hot enough, I find the solution is to lean back into it while on the sofa or the bed.
